Pet Return Form
This form is only for dogs originally adopted from Border Tails Rescue
At Border Tails Rescue, we know how difficult it can be to return your pet. We want you to do everything possible to keep your pet. Once we receive this form, we will be in touch to talk about ways we can try to work through the issues you are having and see if there are any ways we can support you and prevent the need to return your pet. We will always accept animals adopted from Border Tails Rescue back into our care. All returns are by appointment only. An adoption counselor will be in touch within 72 hours to discuss your return form. Once you return a pet, Border Tails Rescue will be unable to provide any updates on that pet to you. We ask kindly that you fill out this form prior to calling us. We are unable to accept returns as walk-ins as kennel space is planned according to what we know is coming in, and we are often times unable to accommodate returns that are not scheduled.
In order for us to provide the best care, we ask that you provide some background information on your pet & their time in your home.

By submitting this application you certify that you own the pet described below and hereby surrender all interest in my pet to Border Tails Rescue. You further state that my pet has not bitten or scratched anyone, breaking the skin, in the past 10 days. You understand that if my pet is adopted, the shelter will not release any of the new owner's information to you. By signing this form, you attest that you are the rightful owner of this animal and agree to comply with all of the statements above.
